Terms of Service
Please read and acknowledge that you understand our billing and cancelation policies.
Appointments will begin with a 1 hour introductory appointment with the purpose to get to know you, determine a diagnosis, and develop a treatment plan. Follow up appointments will be scheduled in line with the treatment plan that you develop with Dr. Runyan. These will range from a 15 minute medication management appointment to a 1 hour psychotherapy appointment and will be scheduled in accordance with the agreed plan. You are the leader of your treatment plan.
We provide hourly based psychiatric care and will bill after service is completed in 15 minute increments at $125 per 15 minutes. This includes offices visits, virtual visits, phone conversations, text messages and paperwork that require the psychiatrist’s time and expertise.
Cancelations made less than 24 hours before a scheduled appointment or failure to attend a scheduled appointment will result in a charge for the full scheduled appointment time. Late arrivals will be accepted but charged based on the scheduled appointment time. If you need to cancel an appointment, please call or text 260-433-16511.
We do not bill insurance directly and are not an in network provider. We will provide a Super Bill only upon request.
We require a credit card to be up-to-date and on file to schedule appointments. Your credit card will be charged at the end of your appointment unless otherwise arranged. Credit card charges will include a 3.5% convenience fee. Cash and check payments are accepted and will be collected at the conclusion of your appointment.
